Note from the reading The Secretary of State's FAQ says regular reporting exists only for certain entities and lists professional associations and LLPs (biennial), nonprofit corporations (continued existence) and name/mark renewals. LLCs are not in the list. The statement is by omission: the page does not say 'LLCs file no report' in those words. An LLC must still keep its registered (statutory) agent current; updates are due within 30 days of a change ($25.00, Form 521).

The rule A general business tax, not an LLC-specific one: 0.26% of Ohio taxable gross receipts. For tax years 2025 and forward only businesses with more than $6 million a year in Ohio taxable gross receipts need to pay it; below that nothing is due and no registration is required. The annual minimum tax ended in 2024.
Not a flat amount: a percentage of Ohio taxable gross receipts, owed only above an exclusion.
